We almost lost our dog a few years ago to something very
similar. He’s a true Hoover, always walks with his nose to the ground
and tastes everything. We were living in MA at the time,
and the poor dog came down with the dire rear, vomiting,
lethargy, etc. We got lucky and rushed him to the vet early on a Friday
morning.

As I said there, I think it’s probably an authentic eggcorn. And it’s interesting to me that it so often turns up as “THE dire rear.” But then again, many people on the Web write “the diarrhea,” which also sounds funny to these Californian ears.
